**** ATTENTION CAMP COACHES **** Camp coaches planning on working this years camps or have already been confirmed to work will need to do the following: * Fill out the Coaches Information form and submit it - CLICK HERE * Print out 1 of the 2 forms listed below. The first form is an authorization form for a background check which we are required to do this year. If you have had a background check done within the last year by your workplace or other please download the second sheet which is an affidavit and have it filled out by your Human Resources Dept. Either sheet will need to be signed and returned to us as soon as possible. THANKS!!! We will need the proper form back as soon as possible. You will not be allowed to work the Camp until we have the form. Once again we are looking forward to having you work our camps this summer!!
